Project narrative: Kidney failure disproportionately affects populations who rely on Community Health Centers for affordable ambulatory care, yet substantial gaps remain in the detection and management of chronic kidney disease. ENRICH will examine kidney disease care and outcomes across a national network of Community Health Centers, identifying multilevel factors and health system transitions that influence care quality and utilization. We aim to map key transitions and identify opportunities to reduce kidney disease progression and, when necessary, ensure patients are optimally prepared for kidney failure.
