Project narrative: People experiencing homelessness have a high burden of chronic disease and die substantially younger than their housed neighbors. Yet little is known about how changes along the housing continuum—including entry into or transitions between housing programs—affect health over time. HOME will link longitudinal housing, health care, social service, and mortality data to examine how these housing transitions influence diabetes and kidney disease outcomes and identify opportunities to improve care for people experiencing homelessness.
